Elevated Concierge Agreement

This Agreement governs your enrollment in an Elevated Concierge program — a 6-month, flat-rate precision wellness partnership. Please read it carefully, including the payment and refund terms, before submitting your enrollment.
1.About This Agreement
By checking the acknowledgment box on your Elevated Concierge enrollment form, you confirm that you have read, understood, and agreed to the terms set forth in this Agreement, in addition to Elevated Life's Wellness Consultation Agreement, which governs the wellness plans, reports, and recommendations you receive as part of your program.
2.What Elevated Concierge Includes
Elevated Concierge is a 6-month, flat-rate wellness partnership built around your AgeCode Premier™ epigenetic results. Your specific plan's included services, testing cadence, and supplement stack are listed on your enrollment page and may be updated from time to time — the current version always governs.
Testing and supplements are coordinated in two phases: your initial AgeCode Premier™ Assessment and first 3-month supplement supply are ordered shortly after enrollment, and your Wellness Essentials Check-In, remaining supplement supply, and eventual retest are ordered 90 days from your enrollment date, timed to arrive before your first supplement supply runs out.
Section 3 — Payment & Refund Policy
Elevated Concierge is billed as a single flat-rate payment for the full 6-month program, payable in full or through Klarna installment financing at checkout.
Because enrollment initiates real costs on your behalf — including your AgeCode Premier™ test kit and supplement shipments — refund eligibility is tied to how much of the program has already been delivered or committed:

Your second round of supplements and remaining tests have not yet been ordered during this window and are fully refundable if you cancel before Day 90 of your program. |

To request a refund or cancellation, contact info@liveelevated.life. Refunds are issued to the original payment method and may take 5–10 business days to appear, depending on your bank or Klarna.
4.Program Term & Cancellation
Your Elevated Concierge program runs for 6 consecutive months from your start date. Canceling after 90 days from your enrollment date ends your active participation in future coordination (new testing, supplement shipments, and scheduled check-ins) but does not entitle you to a refund, as outlined in Section 3.
Elevated Life reserves the right to pause or extend your program timeline in cases of shipping delays, lab processing delays, or other circumstances outside our control, without affecting your refund eligibility under Section 3.
5.Medical Disclaimer
Elevated Concierge is a wellness consultation service, not a medical practice. All testing, supplements, and recommendations provided through your program are governed by the same terms set forth in Elevated Life's Wellness Consultation Agreement — including the requirement that you consult your physician before making any change that could affect an existing medical condition or interact with current medication.
6.Affiliate & Compensation Disclosure
Elevated Life is an affiliate of Genetic LifeSpan™ and an Independent Brand Partner of LifeWave™. These relationships may result in compensation when products or services included in your program are purchased through Elevated Life. This compensation does not influence the wellness recommendations in your plan.
7.Data & Privacy
Your personal information and wellness data are stored securely and used exclusively to build and deliver your Elevated Concierge program. For full details on how your information is collected, stored, and used, please review our Privacy Policy.
8.Limitation of Liability
To the fullest extent permitted by applicable law, Elevated Life and its owners, staff, affiliates, and partners shall not be liable for any health outcome, adverse reaction, or other consequence arising from the use of or reliance on any wellness plan, report, or recommendation provided through your Elevated Concierge program. Your participation is at your own discretion and risk.
