Wellness Consultation Agreement

This Agreement governs your use of Elevated Life's precision wellness consultation services. It applies to both individual clients and Wellness Professionals using the platform. Please read it carefully before submitting any form or activating an account.
1.About This Agreement
By checking the acknowledgment box on your intake or onboarding form, you confirm that you have read, understood, and agreed to the terms set forth in this Agreement.
This Agreement applies to all reports, analyses, wellness plans, and recommendations produced through the Elevated Life platform, regardless of whether you access the platform as an individual client or as a licensed Wellness Professional.
2.What Elevated Life Is
Elevated Life is a precision wellness consultation service. We translate epigenetic data — specifically results from the AgeCode Premier™ biological age assessment — into personalized wellness optimization plans. These plans address lifestyle, nutrition, supplementation, sleep, stress, and daily routine based on your unique epigenetic profile.
Elevated Life is not a medical practice, clinic, hospital, or licensed healthcare provider. We do not employ physicians, nurse practitioners, or other licensed clinical professionals in a patient-care capacity.
3.What Elevated Life Does Not Provide
Elevated Life does not:
- Diagnose, treat, cure, or prevent any medical condition, disease, or disorder
- Prescribe or recommend changes to any prescription medication or clinical treatment protocol
- Provide medical advice, clinical assessments, or second opinions on medical conditions
- Function as a replacement for your primary care physician, specialist, or any licensed healthcare provider
- Establish a physician-patient or therapist-client relationship of any kind
Nothing in any Elevated Life report, plan, or communication should be interpreted as medical advice or a clinical recommendation.
Section 4 — For Clients
This section applies to individuals who have purchased an Elevated Life wellness plan.
You acknowledge and agree that:
Your wellness plan is a personalized guide, not a medical prescription.
The recommendations in your plan — including supplements, foods, lifestyle adjustments, and daily routines — are based on your epigenetic data and are provided for informational and educational purposes only.
You are responsible for your own health decisions.
Elevated Life empowers you with knowledge. How you apply that knowledge is your decision. Before making any change that could affect an existing medical condition or interact with a current treatment, consult your physician or licensed healthcare provider.
Supplements and wellness practices may interact with medications.
Some of the supplements, foods, and natural wellness modalities recommended in your plan may interact with medications or therapies you are currently receiving. You must review any such recommendations with your physician or pharmacist before use. Elevated Life does not assume responsibility for interactions arising from failure to do so.
Your plan is not a substitute for medical care.
If you have an existing medical condition, are under the care of a physician or specialist, or are receiving any form of clinical treatment, your Elevated Life wellness plan is intended to complement — not replace — that care. Always keep your healthcare team informed.
Section 5 — For Wellness Professionals
This section applies to licensed or certified Wellness Professionals who use Elevated Life's platform on behalf of clients.
You acknowledge and agree that:
You are the practitioner of record.
As a licensed or certified health professional, you are solely responsible for the professional care, guidance, and oversight of your clients. Elevated Life is a service provider — it does not enter into a practitioner-client relationship with your clients and does not assume any portion of your professional responsibility.
Elevated Life reports are wellness tools, not clinical assessments.
All reports, plans, and analyses produced by Elevated Life are educational and informational in nature. They are intended to support your practice by providing epigenetically-informed wellness data — not to replace clinical evaluation, diagnostic testing, or treatment planning.
Interpretation and application are your responsibility.
You are responsible for reviewing Elevated Life reports within the appropriate clinical or wellness context of your individual client relationships, determining whether any recommendation is suitable given a client's specific health status, and exercising your professional judgment before sharing or acting on any content.
You must communicate the nature of this service to your clients.
When sharing Elevated Life reports or recommendations with clients, you are responsible for ensuring your clients understand that the content is a wellness consultation — not a medical diagnosis or treatment plan.
Your professional obligations remain fully intact.
Nothing in your use of Elevated Life modifies, limits, or supersedes the obligations imposed by your licensure, certification, or governing professional body. Elevated Life does not assume liability for any clinical decisions made in connection with its reports.
6.Affiliate & Compensation Disclosure
Elevated Life is an affiliate of Genetic LifeSpan™ and an Independent Brand Partner of LifeWave™. These relationships may result in compensation when products or services are purchased through links provided by Elevated Life. This compensation does not influence the wellness recommendations in your plan — all guidance is derived from your individual epigenetic data.
7.Data & Privacy
Your personal information and wellness data are stored securely and used exclusively to build and deliver your Precision Wellness plan. Elevated Life does not sell your data to third parties. For full details on how your information is collected, stored, and used, please review our Privacy Policy.
8.Limitation of Liability
To the fullest extent permitted by applicable law, Elevated Life and its owners, staff, affiliates, and partners shall not be liable for any health outcome, adverse reaction, or other consequence arising from the use of or reliance on any wellness plan, report, or recommendation. Your use of this service is at your own discretion and risk.
